<p>Out-of-hospital cardiac arrest (OHCA) remains associated with a&#xa0;very high mortality rate. Extracorporeal cardiopulmonary resuscitation (ECPR) represents a&#xa0;new potential therapy option and is increasingly being utilized for a&#xa0;selected patient group with refractory out-of-hospital cardiac arrest. Among other prognostic factors, the resuscitation time (low-flow time) until implementation of extracorporeal life support plays a&#xa0;crucial role, linking existing preclinical ECPR concepts to a&#xa0;direct geographical proximity to clinical ECPR centers. The authors describe an ECPR concept for a&#xa0;structurally weak rural district without a&#xa0;regionally available ECPR center, which takes into account the patient-based inclusion criteria and addresses the time-critical nature of the procedure through the early deployment of helicopter emergency medical services.</p>

Out-of-hospital cardiac arrest (OHCA) remains associated with a very high mortality rate. Extracorporeal cardiopulmonary resuscitation (ECPR) represents a new potential therapy option and is increasingly being utilized for a selected patient group with refractory out-of-hospital cardiac arrest. Among other prognostic factors, the resuscitation time (low-flow time) until implementation of extracorporeal life support plays a crucial role, linking existing preclinical ECPR concepts to a direct geographical proximity to clinical ECPR centers. The authors describe an ECPR concept for a structurally weak rural district without a regionally available ECPR center, which takes into account the patient-based inclusion criteria and addresses the time-critical nature of the procedure through the early deployment of helicopter emergency medical services.
