Background <p>The surfaces of furnishings and medical devices or reusable medical products in ambulances represent a&#xa0;potential vector for the transmission of pathogens and must, therefore, be cleaned and disinfected. In this pilot trial, adenosine triphosphate (ATP) bioluminescence testing was investigated as a&#xa0;potential tool for risk analysis and motivation enhancement.</p> Methods <p>Eight observation points were defined and examined on 5&#xa0;days during the morning ambulance shift handover using ATP bioluminescence. The results were discussed directly with the respective crew and served as the basis for the facility-specific definition of assessment thresholds. A&#xa0;spot check was then carried out on 22&#xa0;potentially relevant surfaces and the ambulance crew was asked about the results.</p> Results <p>In all, 27.2% of the surfaces in the spot check showed high exposure, 31.8% showed moderate exposure, and 41% showed low exposure. The surfaces identified as conspicuous were the touchscreen of the documentation tablet computer, the handle of the intravenous (I.V.) set case and the oxygen regulator in the resuscitation backpack, the release switch at the bottom of the stretcher, the skin disinfectant bottle in the I.V. set case in the ambulance and the breathing circuit cover. Our survey suggests that the main reason for inadequate disinfectant cleaning is “negligence due to working routine”.</p> Conclusion <p>The cleaning and disinfection of hand and patient contact surfaces in ambulances contributes to the interruption of potential chains of infection in terms of basic hygiene. ATP measurements can contribute to the identification of relevant surfaces as part of the risk analysis and can be used as a&#xa0;feedback tool to raise awareness and motivate employees as part of a&#xa0;practiced safety culture in the ambulance service.</p>

Background
