Mobiler Computertomograph auf dem Oktoberfest – Stellenwert im Großschadensfall

The Munich Oktoberfest is the world’s most visited folk festival. The demands for the safe execution of this annual mass event are extremely high, both in regular operations and in planning for nonroutine scenarios. In 2022 and 2023, a mobile computed tomography (CT) scanner was placed on the festival grounds for the first time to stabilize clinical and preclinical emergency medicine. It was used to diagnose head and neck injuries, allowing for on-site treatment in most cases. Mobile CT scanners are used in military medicine for diagnostics at the front and for triage. The presence of a fully equipped CT scanner at the Oktoberfest enables its use in a mass casualty incident (MCI). Patients triaged as immediate/red should continue to have the highest transport priority. In the event of an MCI with a high number of patients and insufficient transport capacity, we propose the use of whole-body CT scans (WBCTs) for patients triaged as delayed/yellow. A radiologist and a senior emergency physician should perform a brief assessment. Based on the findings, the response could include life-saving emergency measures, upgrading transport priority, no change, or the decision that transport to a more distant hospital is feasible. We see potential in using the mobile CT scanner at the Munich Oktoberfest during an MCI with a high number of patients and limited transport capacity. This requires special preparation of infrastructure, equipment, and personnel, as well as stringent planning.
