Dual-drug-loaded carbopol gel for enhanced oral wound healing: a curcumin-ciprofloxacin-based therapeutic approach

Chronic oral wounds are prone to infection and delayed healing, necessitating therapeutic gels that combine antimicrobial and regenerative functions.
PurposeThis study aimed to develop and evaluate a dual-drug-loaded oral wound gel (NDWG) incorporating curcumin and ciprofloxacin to assess its antimicrobial, anti-inflammatory, cytocompatibility, and wound healing potential.
Materials and methodsNDWG was formulated using 1% Carbopol 940 as a gel base. The antimicrobial efficacy was tested against S. aureus, E. coli, and K. pneumoniae. The anti-inflammatory activity was measured using an albumin denaturation assay. Degradation and drug release were studied for 28 and 7 days, respectively. Cytocompatibility was evaluated using MTT (3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyl tetrazolium bromide) assays and scratch wound closure in human keratinocytes (HaCaT) cells. A preliminary in vivo evaluation was conducted using the Inflammation-Proliferation-Remodelling (IPR) scale in patients (n = 30) undergoing non-complicated tooth extraction.
ResultsNDWG showed concentration-dependent antimicrobial inhibition, moderate anti-inflammatory activity (~ 60% inhibition at 100 µg/mL), ~ 98% degradation at 28 days, and ~ 95% ciprofloxacin release within 7 days. HaCaT cells maintained > 90% viability with enhanced migration in scratch assays (78–94% closure at 48 h). Clinically, 26 of the 30 patients achieved mucosal closure by day 14, and 90% demonstrated complete remodelling at 6 weeks.
ConclusionNDWG demonstrated promising antimicrobial, anti-inflammatory, and regenerative activities in vitro and favorable preliminary clinical outcomes. However, the findings should be interpreted cautiously given the small in vivo sample size and short follow-up period. Larger, randomized studies are required to confirm their clinical efficacy.
