Objectives <p>To assess trends and their potential driving factors, together with health inequalities regarding the worldwide periodontal diseases burden during the past 32 years.</p> Materials and methods <p>Utilizing the Global Burden of Disease (GBD) 2021 database, we evaluated changes in the periodontal diseases burden between 1990 and 2021, incorporating decomposition analyses to investigate global patterns in prevalence and disability-adjusted life-years (DALYs). Health inequality analyses were conducted through the slope index of inequality (SII) and the concentration index (CI).</p> Results <p>During the study duration, there was a rise of 91.54% in the number of global prevalent cases of periodontal diseases, along with an increase of 90.67% concerning DALYs. Significant heterogeneity was identified in the regional distribution of the burden, with South Asia bearing the heaviest burden in 2021. Population growth and aging represented the dominant driving factors of the evolving cases, comprising a combined share of over 90%. Although countries identified as lower Socio-demographic Index (SDI) faced a heavier burden, there was a decline in absolute inequality related to SDI, as the age-standardized DALYs rate between countries with the maximum and minimum SDI dropped from 50.09 per 100,000 in 1990 to 9.30 per 100,000 in 2021.</p> Conclusions <p>The impact of population aging on the burden of periodontal diseases and the existing absolute inequality in periodontal health deserve widespread attention.</p> Clinical relevance <p>To tackle the challenges posed by periodontal diseases, it is recommended to implement management strategies targeting the aging population and tailor public health policies to mitigate absolute inequality.</p>
