This article reviews current study results on carotid revascularization. It reports on the 2‑year interim analysis of the ECST-2 study on optimized medical therapy and on the CREST-2 trial, which investigates whether carotid artery stenting (CAS) or carotid endarterectomy (CEA), in addition to intensive medical therapy, are superior to intensive medical therapy alone. Further studies examined outcomes following conventional versus eversion endarterectomy, as well as short- and long-term outcomes after transcarotid artery revascularization (TCAR), transfemoral carotid artery stenting (TFCAS), and carotid endarterectomy (CEA). In addition to the postinterventional risk of stroke and death, these studies addressed the incidence of hyperperfusion syndrome, the occurrence of seizures, and the frequency of cranial nerve injuries after the procedure, as well as long-term disability following postinterventional stroke after CEA, TCAR, and TFCAS. Furthermore, reports were presented on redo CEA in asymptomatic and symptomatic patients, on emergency/urgent carotid revascularization, and on bleeding complications after TCAR.
