Diagnostik und Therapie des Ulcus cruris venosum – Zusammenfassung der S2k-Leitlinie

Chronic venous disorders are a frequent disease and venous leg ulcers are the severest form. Therefore, the S2k guidelines on the diagnostics and treatment of venous leg ulcers were published in the guideline register of the Association of the Scientific Medical Societies in Germany (AWMF) last year under the auspices of the German Society of Phlebology and Lymphology. The cause of venous leg ulcers is an increased venous pressure. Moreover, skin changes that are associated with a chronic venous insufficiency, such as hyperpigmentation, lipodermatosclerosis or corona phlebectatica have been described as risk factors for the development of venous leg ulcers. Therefore, duplex sonography should be performed at an early stage. Insufficient superficial veins can be easily identified by duplex sonography, which is called sourcing. Early treatment of the venous reflux is necessary and leads to a faster healing of ulcers. Moreover, the risk of recurrences can be reduced by interventional treatment. Compression therapy is recommended in order to prevent recurrent venous ulcers.
