Von der Karzinogenese zur personalisierten Therapie: der Einsatz von patientenabgeleiteten Organoiden beim Pankreaskarzinom

Pancreatic ductal adenocarcinoma (PDAC) is one of the most aggressive malignant diseases and is associated with an extremely poor prognosis. Due to late diagnosis, high tumor heterogeneity, and pronounced resistance to therapy, survival chances remain limited despite multimodal therapeutic strategies. Personalized therapeutic approaches are therefore urgently needed. In recent years, patient-derived organoids (PDOs) have emerged as a promising preclinical model, as they reflect the morphologic, genetic, and functional characteristics of the primary tumor and allow drug testing as well as the investigation of tumor biology, resistance mechanisms, and biomarkers. Observational studies have demonstrated the potential of PDOs to predict individual responses to treatment and to identify novel drugs and therapeutic strategies. However, methodological challenges, lack of standardization, and insufficient representation of the tumor microenvironment have so far restricted their clinical application. The validation of PDOs in prospective randomized clinical trials and the development of physiologically relevant and standardized models could significantly advance personalized therapy for pancreatic cancer.
