Background <p>Prostate cancer (PC) represents the most frequent invasive cancer among men in Germany, other than skin cancer. One in 7&#xa0;men will be diagnosed with PC up to 85&#xa0;years of age. The risk of being diagnosed with PC is very small before the age of&#xa0;50.</p> Objectives <p>The work presents up-to-date results of the incidence of PC and tumor-related mortality. It further demonstrates capacities of population-based cancer registries to quantify morbidity and mortality resulting from cancer in the population.</p> Materials and methods <p>Cancer registry data were used and included 28,738 men diagnosed with invasive PC between 1974 and 2023 and residing in the German federal state of Saarland. Observed rates of incidence and mortality including average annual percentage change as well as 5‑year observed and cancer-related survival were reported.</p> Results <p>From 2019–2023, 207&#xa0;PC were diagnosed per 100,000 men per year; whereby the incidence rate had increased on average by 1.7% per year in the preceding 50&#xa0;years. Overall, 42% of PC diagnoses were in men up to 70&#xa0;years, 65% of the tumors were limited to the prostate (UICC [Union internationale contre le cancer] stages&#xa0;I and&#xa0;II), and 59% had a&#xa0;Gleason score of ≤ 7a. Five years after diagnosis, 72% of the patients were still alive. Increased tumor-related mortality within 5‑years after diagnosis was observed in patients with PC with UICC stage&#xa0;IV or Gleason score ≥ 8, respectively.</p> Conclusion <p>The analyses revealed the groups of patients with PC encountering increased tumor-related mortality. Implementation of interfaces for electronic data interchange according to the cancer registries’ specifications should become compulsory for software producers and health care providers should use them broadly.</p>
