Oligometastatic disease represents an intermediate stage between a locoregionally confined primary tumor and widespread metastatic dissemination, where targeted local therapies for metastases, in combination with effective systemic treatments, can improve patient survival in selected malignancies. This study explores recent advancements in the management of oligometastatic disease, with a particular focus on the role of stereotactic body radiation therapy (SBRT) for rarer oligometastatic tumors such as head and neck squamous cell carcinoma (HNSCC). Furthermore, innovative developments in SBRT, including the PULSAR (personalized, ultrafractionated stereotactic adaptive radiotherapy) approach and spatially fractionated radiation therapy techniques, are examined. In addition, emerging surgical strategies for the treatment of oligometastases, particularly in colorectal and pancreatic cancer, are discussed. Current evidence regarding the prognostic and predictive value of circulating tumor DNA (ctDNA) and prostate-specific membrane antigen (PSMA)-positive extracellular vesicles in oligometastatic prostate cancer is also analyzed. Finally, the potential of AI-driven algorithms for prognostic assessment and treatment optimization in patients with oligometastatic disease is critically evaluated.
