<p>Oligometastatic prostate cancer remains a&#xa0;clinical challenge, requiring a&#xa0;three-pronged approach: systemic therapy, local therapy, and metastasis-directed treatment. As of 2025, the foundation of systemic therapy for oligometastatic prostate cancer consists of the combination of an androgen receptor inhibitor and androgen deprivation therapy (ADT). Regarding the addition of local therapy, no prospective randomized trial has demonstrated a&#xa0;prognostic benefit when combined with the current standard of systemic therapy. The decision to treat metastases should be made on an individual basis. In the following, we discuss the current evidence for these therapeutic components and provide corresponding recommendations.</p>
