<p>Follicular lymphoma (FL) represents a challenging and stimulating field in oncology, particularly due to dynamic developments in disease management and treatment. In recent years, significant improvements in terms of disease control and reduced toxicity have been seen, mainly due to the development of cell-based and innovative immunotherapies. The current review outlines the standards of care, provides rationale for the treatment strategies, and delves into the latest advancements. The spectrum of therapeutic options is presented in detail, in order to address the variability in disease courses and the individual adaptation of treatment. First we discuss the optimal, curative-intent treatment for early-stage FL, highlighting prominent ongoing studies targeting this patient group as well as potential pitfalls. Secondly, we address the challenges to selecting appropriate first-line treatments for advanced FL, detailing necessary criteria, key studies, and patient-individual factors. Special attention is paid to the management of relapsed and refractory disease, in which the heterogeneity of FL results in significant prognostic differences. We illuminate which treatment options are suitable depending on the recurrent situation, offering guidance for the highly individualized decision between the available approaches—from bispecific antibodies to CAR&#xa0;T-cell therapies. This review aims to provide support for clinical decision-making and present the manifold options for treating FL oriented toward the individual patient.</p>
