The treatment of seminoma and nonseminomatous germ cell tumors in men with stage I disease and in first recurrence is associated with high cure rates. Recurrences should be recognized early in order to avoid progression into higher stages and, thus, more intensive therapies. Chemotherapy, radiotherapy, and surgery should always be applied based on the stage and prognosis group and according to current guidelines. If recurrence occurs after therapy for metastatic disease, conventional salvage and high-dose chemotherapy and autologous stem cell transplantation are standard of care. All forms of therapy are associated with the risk of short- and long-term toxicities, which must be addressed as part of aftercare programs.
