Flächendeckende onkologische Qualitätssicherung durch Landeskrebsregister am Beispiel des Lungen- und Zervixkarzinoms

With the establishment of nationwide cancer registration in Germany based on the Act for the Implementation of the Early Detection of Cancer and Cancer Registries of 2013 (now SGB V, § 65c), the prerequisite for comprehensive oncological quality assurance through state cancer registries has been established. The nationally uniformly structured data from state cancer registries allow for an assessment of care across all cases, irrespective of the care sector or sector boundaries. The aim of this study is to provide an initial overview of cross-regional oncological care using the example of cervical and lung carcinomas.
MethodsData for cervical and lung carcinomas for the diagnosis years 2017–2019 were included from 13 state cancer registries. A total of seven quality indicators (QI) from S3 guidelines were operationalized, calculated, aggregated, merged, and evaluated in provider benchmarking.
ResultsAnalysis included treatment data for 111,857 lung carcinomas with 1898 providers and 8641 cervical carcinomas with 1262 providers. For example, the lung carcinoma QI “Adjuvant Cisplatin-Based Chemotherapy in NSCLC „non-small cell lung cancer“ Stage II” had an average fulfillment rate of 23% across providers. The cervical carcinoma QI “Cisplatin-Based Radiochemotherapy” showed a fulfillment rate of 89%.
DiscussionData from clinical cancer registries enable for the first time to assess the quality of oncological care for the entire population affected and across all providers.
