Glioblastoma is one of the most common primary neoplasms of the central nervous system and has an unfavorable prognosis. The criteria for an integrated diagnosis of glioblastoma are based on histological and molecular characteristics, which are anchored in the current World Health Organization classification. First-line postoperative therapy consists of a combination of radiotherapy, temozolomide chemotherapy (with or without lomustine, depending on the molecular tumor profile), and tumor treating fields. Upon tumor progression, treatment recommendations are based on various criteria, including clinical status, time elapsed from first-line therapy, and the pattern of disease progression. Therapeutic clinical trials are an integral part of neuro-oncological care at all stages of glioblastoma. Current strategies in clinical trials include biomarker-based and immunotherapies, as well as optimization of current treatment concepts.
