Intestinal malrotation is a rare congenital malformation in which the large and small intestines are malpositioned due to incomplete rotation of the gastrointestinal tract during embryogenesis. As a result, the clinical presentation of acute abdominal conditions can be significantly altered. While the spectrum of intestinal malrotation ranges from asymptomatic cases to life-threatening complications such as neonatal volvulus, diagnosis often remains a challenge, particularly in adults. We present a case of a young man in whom malrotation led to a delayed diagnosis of acute appendicitis. This case illustrates how anatomical variations can lead to a delay in diagnosis and in the initiation of appropriate treatment.
