Congenital valvular heart disease in adults with congenital heart defects (ACHD) represents a growing challenge in modern cardiovascular medicine. The increasing number of adults with congenital heart defects (ACHD) brings into focus the long-term sequelae of valvular abnormalities, many of which require highly individualized and often multidisciplinary approaches. Anatomical alterations due to previous surgery, chronic biomechanical stress on the valves, and age-related degenerative changes contribute to the clinical complexity. Catheter-based interventions have become indispensable therapeutic options. Transcatheter pulmonary valve implantation (TPVI) is now considered the standard treatment for dysfunctional pulmonary valves or conduits in the right ventricular outflow tract. Additionally, valve-in-valve procedures and edge-to-edge repair techniques for systemic atrioventricular valves are significantly expanding the therapeutic armamentarium. Treatment success depends largely on the experience of the heart team, interdisciplinary collaboration, and meticulous preprocedural planning.
