Reviews on the topic of “stroke in young adults” now describe more than 150 etiologies of stroke in younger people; thus, the diagnosis of a stroke is quite complex. Nevertheless, the well-stablished TOAST (Trial of Org 10172 in Acute Stroke Treatment) criteria remain valid, which classify stroke into five major groups, including stroke due to “large artery arteriosclerosis”, “cardioembolism”, “small vessel occlusion”, “stroke of other determined etiology”, and “stroke of undetermined etiology”. Combining these literature reports shows that “stroke of other determined etiology” is the most complex to address in clinical routine. What is common enough to be included in a routine work-up, especially if the affected person is no longer considered “young”, which in stroke studies is typically defined as those younger than 50 years of age. Our case report describes exactly such a person and is intended to show which indications can be used in daily routine to expand the diagnostic workup.
