Severe stroke is a huge challenge—not only for patients, but also for families and their treatment teams. Appropriate goals of care according to individual goals, preferences and values often have to be developed under time pressure. Limited or lacking decision-making capacity and the need for surrogate decisions pose additional challenges. Families in this situation need understandable, consistent information, emotional support and a professional team that recognizes their needs and supports them in the decision-making processes. All professionals working in stroke care need these skills. Specialized palliative care services provide important support.
