<p>This article examines, from the patients’ perspective, barriers and facilitators influencing access to outpatient psychotherapy in Austria, with attention to possible urban–rural differences. The analysis is based on 43&#xa0;interviews with patients from private practices, using qualitative content analysis. Central barriers identified were a&#xa0;complex reimbursement system, distressing experiences with healthcare professionals, and negative previous psychotherapy experiences; in some cases, feelings of shame were also reported. Patients also described facilitators such as social support, professional guidance, and positive previous therapy experiences. The findings highlight the need for a&#xa0;simplified and better-funded reimbursement system, improved information provision, and measures to reduce stigma. Frequent ambivalent prior experiences with psychotherapy warrant further research.</p>
