Autonomie und Fürsorge im Spannungsfeld: Eine kritische Betrachtung des assistierten Suizids bei Komplexer Posttraumatischer Belastungsstörung

Complex posttraumatic stress disorder is a chronic condition, which usually requires long term support. Symptoms can be reduced to a certain extent, but complete recovery is unlikely. Affected patients usually have a long history of suffering, involving lengthy therapeutic processes. Their lives are continuously impacted by the cumulative effects of setbacks, trauma, mistrust, hopelessness and a dysfunctional outlook on life. Their increased vulnerability fosters further trauma over the life span and is often accompanied by chronic suicidality.
On January 1st, 2022, assisted suicide became a legitimate option in Austria, when a new law, the “Sterbeverfügungsgesetz”, came into effect. How can psychotherapists handle their patients’ desire for assisted suicide in cases of complex posttraumatic stress disorder? The authors examine the therapist’s role in these specific situations, illustrated with a case study. The necessity to uphold patient autonomy while simultaneously fulfilling the duty to prevent suicide leads to a tension that raises both ethical and professional questions. Using current studies, this paper examines this dilemma.
