<p>The introduction of targeted biologics (e.g., anti-IL-4/13 antibodies such as dupilumab, anti-IL-12/23, anti-TNF-alpha antibodies) and oral Janus kinase inhibitors (JAKi) have revolutionized the treatment of severe dermatological diseases in children in recent years. However, intervening in the child’s immune system also raises specific questions regarding vaccination planning and potential interactions, particularly concerning immunogenicity, safety (especially of live vaccines), and the timing of pre- and post-initiation therapy. This review summarizes current evidence and recommendations and aims to provide practical guidance for pediatrics and pediatric dermatology.</p>
