Background <p>Menstrual cycle disorders are a&#xa0;common gynecological problem in adolescents. Increased menstruation (heavy menstrual bleeding, HMB) in the form of hypermenorrhea and menometrorrhagia are a&#xa0;clinical challenge and can significantly impair the quality of life of young women. While most cases are due to physiological immaturity of the hypothalamic–pituitary–ovarian axis, other pathological causes—particularly congenital bleeding disorders—must not be overlooked.</p> Objective <p>This review aims to summarize diagnostic pitfalls and treatment strategies for HMB in adolescents, with a&#xa0;particular emphasis on the recognition and treatment of congenital bleeding disorders and the individualization of therapy to the specific needs of adolescent patients.</p> Methods <p>We have summarized current literature, guideline recommendations, and illustrative clinical cases to provide a&#xa0;structured overview of diagnostic and therapeutic approaches for HMB in adolescents.</p> Results <p>The majority of HMB cases in adolescents are due to anovulatory cycles. Up to 20% of affected adolescents have an underlying bleeding disorder, most commonly von Willebrand disease, thrombocytopenia/thrombopathy, or a&#xa0;bleeding disorder of unknown cause. A&#xa0;careful family and individual bleeding history, supported by targeted laboratory diagnostics, is crucial to identify the cause. Treatment includes supportive measures such as iron supplementation, acute interventions such as high-dose hormone regimens and blood products, as well as long-term options such as combined hormonal contraceptives and hormone-releasing intrauterine devices. Individualized, age-appropriate diagnostic and treatment plans are essential for the quality of life of affected adolescents.</p> Conclusion <p>HMB in adolescents requires a&#xa0;structured diagnostic approach and individualized treatment plans. Increased awareness of the consistent application of guidelines and evidence-based diagnostic and therapeutic measures can improve the quality of life of affected patients.</p>

Background

Menstrual cycle disorders are a common gynecological problem in adolescents.
