Adolescents with gender incongruence are vulnerable and often have accompanying mental disorders. Counseling should include the involvement of legal guardians in the treatment process. Psychotherapy to promote self-esteem, support identity development, and treat accompanying disorders should be offered on a low-threshold basis. Support for adolescents from their environment can improve the prognosis for the mental health of gender-incongruent adolescents and is the focus of counseling. Graduated medical treatment can reduce physical dysphoria and contribute to improving mental health. A careful diagnostic process and a risk–benefit assessment on a case-by-case basis, in accordance with ethical principles and taking into account national and international guidelines, are required to determine the indication.
