A 15-year-old male adolescent with known systemic lupus erythematosus and HIV infection presents with a 4-day history of malaise, fever, and cough. Due to his chronic conditions, the patient is under a immunsuppressive and antiretroviral therapy, which he only takes inconsistently. Laboratory work-up yields elevated inflammatory markers. The chest radiogramm showes a pathologic finding for which the patient is admitted to the hospital. Further diagnostic tests are ordered, and an empiric treatment is started. Still, the patient’s condition worsens, and he develops a pneumothorax, necessitating drainage and an intensive care unit admission.
