<p>This case report describes a new approach to C2 vertebroplasty in a 70-year-old female with a C2 myeloma lesion with potential risk of fracture. Traditional anterolateral and transoral approaches were contraindicated due to a medialized carotid artery. An innovative direct posterolateral approach was used, combining over-the-wire and hydrodissection techniques bypassing the vertebral artery, achieving effective stabilization with no complications.</p>
