<p>Polycystic ovary syndrome (PCOS) is a complex and heterogeneous condition characterised by a range of clinical and biochemical features, including hyperandrogenism, ovulatory dysfunction and polycystic ovarian morphology. This study aimed to evaluate the levels of irisin, a thermogenic hormone associated with energy metabolism, in women with PCOS compared to healthy controls. The study included 96 women with confirmed PCOS and 48 age-matched healthy controls, all within reproductive age. Anthropometric measurements, hormonal evaluations and blood tests were conducted to assess irisin levels, metabolic markers and liver function. PCOS patients had significantly higher weight and BMI than the control group. The mean irisin level was considerably higher in the PCOS group (6.757 ± 0.15), compared to the control group (6.172 ± 0.17), with a <i>p</i>-value &lt; 0.05. Hormonal assessments revealed elevated levels of luteinising hormone (LH), follicle-stimulating hormone (FSH), thyroid-stimulating hormone (TSH), prolactin (PRL) and free testosterone (FRTST) in PCOS patients. Fasting blood glucose (FBG), liver enzymes (AST and ALT), total cholesterol (TC) and triglycerides (TG) were also significantly higher in the PCOS group, compared to controls. A significant positive correlation was found between FRTST levels and irisin in women with PCOS. In contrast, a significant negative correlation was observed between serum irisin levels and BMI in PCOS patients. The study concludes that women with PCOS exhibit elevated irisin levels, which are associated with metabolic disturbances, including liver function abnormalities and lipid profile disorders. These findings suggest that irisin could serve as a potential biomarker for metabolic complications in PCOS, providing a new avenue for research and potential clinical applications.</p>
