<p>The exposure to forests is attributed with various positive effects on mental, physical and social health. Consequently, forest landscapes are increasingly utilized not only for disease prevention (as recreational spaces) but also therapeutically, being integrated into and researched within various treatment concepts. While internationally published studies on forest therapy primarily focus on the therapeutic effects in terms of symptom reduction among treated patients or analyze the therapeutic process within natural settings, the natural characteristics of therapeutic forests and the infrastructural requirements related to forest therapy are rarely addressed; however, these aspects represent important prerequisites for effective and safe therapy. The present study investigates which forest characteristics and infrastructural needs promote such therapy in the forest with particular consideration of the perspectives of patients who have been treated in the context of multimodal pain therapy in forest settings.</p><p>From the patients’ perspective light, bright mixed forests are particularly suitable therapeutic environments and infrastructural measures in the forest should be implemented to address the pronounced need for safety among these patients. These necessary therapy-related interventions in the natural forest environment highlight future challenges that require interdisciplinary collaboration among medicine, forestry and spatial planning. As the current trend of (re)utilizing forest recreational landscapes as therapeutic spaces increases, such interdisciplinary concepts that take the patients’ perspectives into account will gain greater importance.</p>
