Background and aims <p>Endoscopic suturing using a conventional suturing device has a steep learning curve. This study assesses the ease of use of a novel endoscopic suturing system compared to the conventional device using a validated simulator.</p> Methods <p>This randomized, crossover study included suturing experts and novices performing pursestring suturing on a transoral outlet reduction simulator. Subjects were randomized to use the novel or conventional device first. Simulator scores and workload using National Aeronautical and Space Administration Task Load Index were recorded.</p> Results <p>Ten subjects participated (4 experts, 6 novices). The novel device achieved higher simulator scores (179.3 ± 73.7 vs. 99.8 ± 94.1; P = 0.049) and lower workload (39.2 ± 15.9&#xa0;vs. 70.5 ± 22.1; P = 0.002) than the conventional device. Novices demonstrated the greatest benefit.</p> Conclusion <p>This novel endoscopic suturing system improves ease of use and reduces workload, especially in endoscopists without prior suturing experience.</p>
