<p>Inflammatory pseudotumours (IPTs) are benign lesions that rarely occur in the liver, usually presenting as mass-forming lesions characterized by bland spindle cells intermingled with a mixture of inflammatory cells, etiologically related to inflammatory response caused by infection and trauma. The combination of hepatic IPTs with primary liver malignancies is anecdotal to our knowledge. This study reports a case of hepatic IPT synchronously arising at the periphery hepatocellular carcinoma (HCC) in a 66-year-old male patient who had 2 liver biopsy procedures before undergoing surgical resection. The occurrence of IPT in this case is perceived as a reactive inflammatory response due to traumatic event (prior biopsy procedures) and fungal infection (<i>Cystobasidium slooffiae</i>) after conducting a thorough evaluation at the histological, immunohistochemical, and molecular levels to rule out the potential differential diagnoses and to identify any infectious agents.</p>
