Purpose <p>To analyze whether disease duration and severity can predict voice quality improvement following office-based blue laser therapy (OBLT) for benign lesions of the vocal folds.</p> Study design <p>Retrospective chart review.</p> Methods <p>Medical records of patients with benign lesions of the vocal folds who underwent OBLT between November 2023 and December 2025 were reviewed. Demographic data included age, gender, history of smoking, reflux disease, voice abuse, diagnosis, follow-up period, and disease duration. Self-perceived disease severity was assessed using the Voice Handicap Index-10 (VHI-10) and the Visual Analog Scale (VAS). Secondary outcome measures included acoustic and aerodynamic parameters.</p> Results <p>Sixty patients were included (35 females, mean age: 52.35 ± 14.45). The mean disease duration was 15.35 ± 20.80 months. Linear regression analysis showed no association between disease duration and mean change in VHI-10 (<i>r</i> = 0.018, <i>p</i> = 0.901) or VAS scores (<i>r</i> = 0.021, <i>p</i> = 0.886). There was a strong significant association between pre-operative VHI-10 score and mean change in VHI-10 (<i>r</i> = 0.681, <i>p</i> &lt; 0.001) and the mean change in VAS scores (<i>r</i> = 0.636, <i>p</i> &lt; 0.001), and between pre-operative VAS scores and mean change in VHI-10 (<i>r</i> = 0.642, <i>p</i> &lt; 0.001) and the mean change in VAS scores (<i>r</i> = 0.758, <i>p</i> &lt; 0.001). A weak positive significant association was found between pre-operative VAS scores and mean change in percent jitter (<i>r</i> = 0.271, <i>p</i> = 0.040). No significant associations were observed between disease duration, disease severity, and other acoustic or aerodynamic parameters.</p> Conclusion <p>Disease severity and not disease duration was associated with self-perceived improvement in voice quality following office-based blue laser therapy for benign vocal fold lesions.</p>
