Scalp biophysical characteristics of healthy women wearing hijab compared to those who do not: a cross-sectional study

The scalp is the anatomical area characterized by high density of terminal hair growth with numerous sebaceous glands that contribute to a specific microenvironment with significant differences from the rest of the skin. The regular and prolonged use of hijab, a type of headdress worn by some Moslem women typically covering the whole scalp area extending to the neck, could interfere with this specific environment, especially its biophysical characteristics, and may be a trigger to several dermatoses. This study aims to identify and compare the biophysical characteristics of the scalp of healthy women wearing hijab to those who do not. This was a cross-sectional study conducted from August 2019 to April 2020. A total of 188 premenopausal women aged 18 years or older were recruited. The study protocol has been registered in clinicaltrials.gov (NCT05267119). Subjects in hijab group had worn hijab for a minimum duration of 8 h a day for at least 5 years. After a wash out period, transepidermal water loss (TEWL), skin capacitance (SCap), and sebum levels were examined. Obtained data was analyzed using Mann-Whitney test. No significant differences of TEWL (Z = 0.815, p = 0.42), SCap (Z = 0.271, p = 0.79), and sebum levels (Z = 0.698, p = 0.49) were found between the groups. This study was conducted on a small sample size. Moreover, this study did not assess hair thickness and density because the required equipment, such as TrichoScan, was not available in our institution. Despite these limitations, this data provides valuable insights into the biophysical characteristics of the scalps of healthy women who wear hijabs. There were no differences in scalp biophysical characteristics in terms of TEWL, skin hydration, and sebum levels between women wearing hijab and those who did not. Further studies should be conducted in other populations with a larger sample size to confirm these results.
