Superficial leiomyosarcoma: a systematic review of treatment with mohs micrographic surgery

The current recognized treatment for superficial leiomyosarcoma (LMS) is wide local excision (WLE), with surgical margins ranging from 2 to 5 cm. The present literature on WLE reveals variations in the efficacy of this treatment modality. Given the established role of Mohs micrographic surgery (MMS) for other cutaneous tumors it is important to define its potential role for LMS. We conducted an exhaustive review of the literature utilizing PubMed and the Wiley online library, covering works from the databases’ inception to January 2025. In a systematic review of 73 recorded LMS cases treated with MMS, of which the majority were dermal LMS, our results showed consistently low rates of recurrence, metastasis, and combined recurrence and metastasis. MMS may be a promising option for management of dermal LMS. Our study was limited by the number of subcutaneous cases in the literature, limiting conclusions to dermal LMS. While our results must be contextualized with the variable recurrence rates reported for WLE, these findings provide the groundwork for larger scale studies. It is critical that investigators continue to compare these two treatment modalities with larger sample sizes in order to comprehensively assess their effectiveness in managing LMS and determine the most effective treatment option for LMS.
