<p>The care of patients on the heart transplantation waiting list represents a&#xa0;complex and dynamic phase in the course of advanced heart failure. It requires a structured, interdisciplinary management aimed at maintaining transplantation eligibility, detecting clinical deterioration at an early stage and responding with timely therapeutic interventions. In addition to optimizing drug treatment, regular clinical and instrumental re-evaluations, monitoring of end-organ function and assessment of the patient’s hemodynamic stability play a&#xa0;central role. Particular importance is attached to the early identification of complications and the escalation of treatment, including temporary and/or permanent mechanical circulatory support. Furthermore, organizational aspects, such as the actuality of the waiting list data, close collaboration with transplantation coordination and preparation for a&#xa0;potential organ donor are essential. Psychosocial factors and treatment adherence substantially influence the course on the waiting list and should be an integral part of patient management. This article provides a&#xa0;practice-oriented overview of the key medical, organizational and interdisciplinary aspects of the care for heart transplantation candidates on the waiting list.</p>

The care of patients on the heart transplantation waiting list represents a complex and dynamic phase in the course of advanced heart failure. It requires a structured, interdisciplinary management aimed at maintaining transplantation eligibility, detecting clinical deterioration at an early stage and responding with timely therapeutic interventions.
