<p>We systematically reviewed the cardiac surgery literature on heart valve diseases from 2024 and compiled a&#xa0;summary.</p><p>Last year, a&#xa0;debate was sparked about the relative value of evidence from randomized trials vs. evidence from registry data, with conflicting results for structural heart diseases.</p><p>In aortic stenosis the early advantage of catheter-based (transcatheter aortic valve implantation, TAVI) treatment over surgical treatment (surgical aortic valve replacement, SAVR) found further support; however, the long-term results of TAVI remain controversial. In 2024 criticism arose due to new FDA and registry data favoring SAVR. When aortic valve replacement was necessary again, surgical reintervention (redo SAVR) showed better results compared to valve-in-valve TAVI.</p><p>In the area of mitral valve diseases, studies showed short-term noninferiority of catheter-based treatment compared to surgery in secondary mitral regurgitation. At the same time, however, registry data showed a&#xa0;large long-term survival advantage for surgical treatment in primary mitral regurgitation. Finally, in acute type&#xa0;A intramural aortic hematoma, surgical treatment was associated with a&#xa0;better survival rate compared to pharmacotherapy.</p><p>This article summarizes publications from the past year that we consider important and is therefore not free from subjective interpretation; however, it provides up to date information that may be helpful for patient-specific decision-making.</p>
