<p>Endoscopic cardiac surgery enables complex cardiac procedures through minimal access routes using specialized endoscopic instruments and visualization systems. These techniques have become established as a&#xa0;valuable alternative to conventional cardiac surgical procedures via median sternotomy, aiming to reduce invasiveness while completely ensuring procedural safety and efficacy. This article outlines the current state of endoscopic cardiac surgery, including operative techniques, clinical fields of application and key challenges in the implementation in surgical training. The reported results of endoscopic cardiac surgery are highly promising, with mortality and complication rates comparable to conventional approaches, alongside the benefits of reduced surgical trauma and faster postoperative recovery. Endoscopic procedures in cardiac surgery have so far not been completely accepted in routine clinical practice. Nevertheless, in the future this field will be extensively influenced by technological innovations, robotic systems and artificial intelligence, which are expected to further refine the precision and expand the scope of indications.</p>
