<p>Multidrug-resistant pathogens are an increasing problem in cardiac, thoracic and vascular surgery. Immunosuppressed patients and infections associated with implants are particularly affected as the risk of implant removal is often too high. As antibiotic treatment is losing effectiveness, the World Health Organization (WHO) calls for innovative treatment strategies. A&#xa0;promising alternative is phage treatment, which selectively lyses bacterial pathogens. Initial clinical reports indicate good efficacy and tolerability, especially for antibiotic-resistant infections. In western countries, phage treatment is still used off-label and requires individual case decisions. While already established in Belgium and France, its development in Germany is still in the developmental phase. Despite regulatory hurdles, research on phage safety, efficacy and production is increasing. This article provides an overview of the current status of phage treatment in Germany, its clinical evidence in cardiac and vascular surgery and potential recommendations for the clinical implementation.</p>
