Radiology plays a crucial role in both the initial assessment and subsequent monitoring of the position and integrity of cardiac rhythm devices. Among imaging modalities, chest radiography holds a unique position as it enables the evaluation of the physical integrity of leads used in these devices in a simple and low-threshold manner. Therefore, a fundamental understanding of the typical and atypical radiographic appearances of these devices and their various components is essential. Acute complications following the implantation of cardiac rhythm devices encompass cardiac dysrhythmia, pneumothorax, perforation of cardiac or venous structures, mechanical damage of heart valves, lead damage, improper seating of the terminal connector pin and the presence of air pockets in the pacemaker. Long-term complications include twiddler syndrome, lead fractures, damage to lead insulation, and lead displacement. This article presents the implants used and the most frequent disorders/complications and aims to serve as a basis for a rapid assessment.
