The patency rate of the mammary artery is superior to all other bypass grafts in coronary surgery. This article describes complete arterial revascularization using the bilateral mammary artery as a universal procedure in coronary surgery, including the T‑graft and I‑graft technique. In addition to possible contraindications, details of the surgical protocol and the procedure with limited mammary artery material are described. Special features of anticoagulation in off-pump coronary surgery are also highlighted.
