Long-term cognitive benefits of human milk phospholipids in breastfed children at preschool age: results from MUAI study

To examine the associations between human milk phospholipids (PLs) concentrations and neurocognitive development in preschool-aged children.
MethodsThis study was part of the maternal nutrition and infant investigation (MUAI) study. Human milk PLs concentrations were previously determined, and children’s cognition were assessed using the Griffiths Mental Development Scales-Chinese Edition. A total of 65 participants with both milk PLs and cognitive data were included and divided into two groups based on the postpartum timing of milk samples collection: 40–45 days (n = 27) and 200–400 days (n = 38).
ResultsFor PLs classes in human milk, no significant associations were observed with developmental quotients (DQ) in the 40–45 days group. However, in the 200–400 days group, phosphatidylethanolamine (PE) and phosphatidylcholine (PC) concentrations were positively associated with personal-social DQ, with PE also associated with hand–eye coordination DQ. For polyunsaturated fatty acids-containing PLs (PU-PLs) in human milk, polyunsaturated PC (PU-PC) concentration was positively associated with performance DQ in the 40–45 days group. In the 200–400 days group, PU-PLs, including polyunsaturated PE, PU-PC, polyunsaturated lyso-phospholipids, were positively associated with DQs in multiple subdomains. Similar associations were also observed for odd-chain fatty acids-containing PLs and plasmalogens with personal-social DQ. Stratified analysis indicated higher PLs concentrations were associated with increased DQs in preschoolers breastfed for at least 12 months or exclusively breastfed for 6 months.
ConclusionHigher concentrations of human milk PLs, particularly PU-PLs, were associated with improved neurocognitive development in pre-school aged children. These findings underscore the importance of specific PLs in supporting long-term cognitive development and provide insights for future research and human milk substitutes guidelines.
