摘要

Background

Although psychoses were long considered to be a contraindication to psychotherapy, in recent years there has been growing evidence for the effectiveness of psychotherapy for individuals with psychoses. This is also reflected in a change in the psychotherapy guidelines, which now recommend psychotherapeutic interventions in all phases of the illness.

Objective

This article provides an orienting overview of psychodynamic concepts and therapeutic approaches for individuals with psychoses. It also refers to current studies that focus on these concepts.

Material and methods

In addition to the critical presentation and reflection of conceptual works in the field of psychodynamic psychotherapy, the evidence situation is also outlined taking different therapeutic orientations into account. The selection of concepts presented aims to be easily accessible.

Results

Key concepts of psychodynamic psychotherapy for psychoses describe, among other things, a vulnerability of the ego boundaries, fragmentation, a dilemma of interpersonality and peculiarities in the field of symbolization. The approach places particular emphasis on a model experience within the therapeutic relationship, in which interpersonal tensions can be reduced.

Conclusion

Psychodynamic psychotherapy offers a range of theoretical approaches that can be valuable for understanding and treating individuals with psychoses. Although initial studies attempt to test central assumptions of the theories, the evidence base so far appears to be insufficient and further research is needed on the effectiveness of psychodynamic psychotherapy and on the specific influencing factors.
