Die Rolle der Psychotherapie in klinischen Studien mit Psychedelika

In recent years there has been a notable surge in clinical trials investigating classical and atypical psychedelic drugs for the treatment of various psychiatric indications. Of these two substances (N-methyl‑3,4‑methylenedioxyamphetamine, MDMA, psilocybin) have progressed to the advanced phase 3 stage of clinical drug testing, the last phase before potential regulatory approval. What distinguishes the therapeutic utilization of psychedelic drugs is the close interconnection with psychotherapeutic components. This narrative review provides an overview of typical interventions based on selected studies and elucidates shared characteristics and distinctions within the therapeutic paradigms applied in contemporary clinical investigations involving psychedelic drugs. The fundamental therapeutic framework underpinning most studies involving psychedelic drugs comprises three essential phases: preparatory, substance administration and follow-up sessions. Notably, this framework defies categorization within any particular psychotherapeutic school and is not disorder specific. Within this model therapists assume the pivotal role of establishing a secure environment for patients to navigate psychedelic experiences, while also providing guidance in processing these experiences. The therapeutic approach is predominantly nondirective and characterized by a supportive stance. Some studies combine this basic framework with established therapeutic procedures, which often forms a disorder-specific augmentation. Even if the overall significance of nonpharmacological elements in psychedelic treatment remains indisputable, there exists a dearth of comprehensive understanding regarding the optimal design of psychotherapeutic components. Presently, large-scale clinical trials primarily emphasize the demonstration of the efficacy and safety of psychedelic-assisted therapy, as this empirical evidence is imperative for regulatory drug approval; however, to further enhance the refinement of these innovative treatment modalities, future research endeavors will necessitate increasingly more elaborate study designs, enabling nuanced exploration of diverse psychotherapeutic interventions.
