Pretarsal Fibro-adipose Fascial Layer in Congenital Single and Double Eyelids: A Cadaveric, Histologic, and Ultrasonographic Study

To explore the relationship between the pretarsal fat fascia layer and upper eyelid morphology and to guide the reconstruction of bionic natural eyelid blepharoplasty.
MethodsThe structure of the pretarsal fat fascia layer of congenital single eyelid and double eyelid was observed by ultrasound, intraoperative observation, cadaver dissection, and histological staining.
ResultsBy ultrasonic, intraoperative, anatomic, and histological analysis, the pretarsal fat fascia layer of congenital single eyelid and double eyelid were divided into two layers: superficial loose fat fascia layer and deep dense fascia layer. The dense fascia layer was close to the surface of tarsus, and there was no significant difference between groups A and B. The thickness and volume of the pretarsal loose fat fascia layer were significant difference. It showed the loose fat fascia layer was loose in texture, continuous with the retro-orbicularis oculus fat (ROOF) upward and covered the surface of tarsus downward, and the deep dense fascia layer clung to the surface of tarsus, and connected upward to the fusion of orbital septum and levator palpebrae superioris (LPS) aponeurosis.
ConclusionThe pretarsal fat fascia layer of single and double eyelid can be divided superficial loose and deep dense fat fascia layer. The pretarsal loose fat fascia layer may be one of the main tissue structure factors affecting the double-eyelid blepharoplasty. The treatment of pretarsal loose fat fascia layer may be beneficial to the shaping of the bionic double eyelid.
