Modified Scalp Preserving Coronal Forehead Lift

Eyebrow and forehead lift is a popular facial rejuvenation method, but a key challenge is a predictable low-complication procedure with long-lasting results. Classic coronal forehead lift involves scalp resection, which results in loss of valuable hair follicles.
ObjectivesThis study presents a modified approach to the classic coronal forehead lift, aiming to preserve the scalp while achieving durable results.
MethodsA retrospective review was conducted on 40 patients (33 males, 7 females) who underwent the procedure between March 2021 and June 2023. The technique involved an incision 2-3 cm behind the hairline, divided into three parts: two lateral temporal and one central frontal. The hair-preserving dissection was performed at the deep temporalis fascia and subgalea plane, extending into the brow fat pad. Fixation was achieved through suspensory sutures for eyebrow positioning, and internal net sutures were used over the temple and frontal areas. Postoperative care included a light dressing for 24 hours without drain fixation.
ResultsThe study found high patient satisfaction (52.5% completely satisfied). The most common complications were paresthesia (57.5%) and transient pruritus (47.5%), with no major complications or alopecia observed at the incision site. Additionally, 85% of patients underwent frontal bone contouring and reshaping.
ConclusionThe open coronal brow lift, often overlooked in favor of newer endoscopic techniques, offers predictable and stable aesthetic results. This article advocates for a renewed appreciation of this time-tested procedure, emphasizing scalp preservation.
