<p>We report a very rare case of an intraossesous extra-axial chordoma (EAC) involving the distal femur. We describe the clinical presentation, radiologic and pathological findings, and the treatment. In addition, a systematic literature search was performed to further expand the knowledge regarding EAC in the lower extremities, including 13 papers describing a total of 21 adult patients.</p>
