<p>To investigate the safety and efficacy of intelligent pressure-control system with flexible and navigable suction ureteral access sheath (FANS-UAS) combined with high-power holmium laser during flexible ureteroscopy for the treatment of &gt; 2&#xa0;cm renal stones.&#xa0;Thirty-five patients were included. All patients underwent flexible ureterorenoscopy (f-URS) by utilizing the intelligent pressure-control system and FANS-UAS with a high-power holmium laser. Perioperative data, intraoperative irrigation fluid rate and intrarenal pressure (IRP) were recorded and analyzed.&#xa0;The median stone diameter and volume were 28.0&#xa0;mm and 2423.9&#xa0;mm³, respectively. The median stone density was 1212.0 HU. The median operative time and postoperative hospital stays were 55.0&#xa0;min and 1&#xa0;day, respectively. The median irrigation flow rate was 150.7&#xa0;ml/min and the median IRP was 16.4 mmHg. The immediate SFR was 91.4% and the 3-month SFR was 97.1%. Twenty-six patients (74.3%) achieved complete clearance without residual fragments, and 6 patients (17.1%) had fragments smaller than 2&#xa0;mm. One patient (2.9%) experienced postoperative fever and there were no cases of serious infections and other severe complications.&#xa0;Intelligent pressure-control system and FANS-UAS combined with high-power holmium laser during flexible ureteroscopy were effective and safe in the treatment of &gt; 2&#xa0;cm renal stones, providing a new option for the minimally invasive treatment of complex renal stones.</p>
