Background <p>Deep plane facelift is increasingly used for facial rejuvenation due to its natural and durable results. However, data on patient satisfaction and the influence of comorbidities on outcomes remain limited.</p> Methods <p>A retrospective cohort study was conducted including 53 patients who underwent deep plane facelift between January 2023 and November 2024. Demographic variables, comorbidities, and postoperative complications were analyzed. Patient satisfaction was assessed using the FACE-Q “Satisfaction with Decision” questionnaire. Statistical analyses were performed to evaluate associations between clinical variables, complications, and satisfaction outcomes.</p> Results <p>Thirty-four patients completed the FACE-Q questionnaire. The mean satisfaction score was 78.23 ± 23.01, with most patients reporting high satisfaction. Hypertension was significantly associated with postoperative complications (<i>p</i> = 0.016). Diabetes mellitus was associated with seroma formation (<i>p</i> = 0.038), but not with overall complications. No significant associations were found between smoking or body mass index and complication rates. Satisfaction scores were significantly higher in patients operated on in 2024 compared to 2023 (<i>p</i> = 0.018). No correlation was observed between satisfaction and age or body mass index.</p> Conclusions <p>Deep plane facelift is a safe and effective technique associated with high patient satisfaction. Hypertension and diabetes mellitus may influence specific complications, and surgical experience may contribute to improved outcomes. Level of Evidence: Level IV, risk / prognostic study.</p>
