Upward vector observed in all 17 cases after non-absorbable polyester–silicone thread lift: a vectra 3-D pilot study

Non-absorbable threads are expected to provide durable facial lifting, but three-dimensional volumetric data—particularly for polyester–silicone threads—remain scarce. This study aimed to quantify volumetric lift and short-term retention using Vectra 3D.
MethodsIn this prospective pilot study, 17 consecutive patients underwent thread lifting with non-absorbable polyester–silicone threads placed from a single preauricular entry with bidirectional fixation toward the marionette/oral commissure region and the temporal scalp. Using predefined parallelogram-shaped regions of interest (ROIs), the primary outcome was the relative volumetric difference between the cheek (Zone 2) and perioral region (Zone 1), ΔV(Zone2–Zone1, mL), derived from Vectra H2® Volume Assessment at baseline, 1 month, and 3 months. ROI placement and volumetric measurements were independently performed by two physicians, and the mean of the two measurements was used for analysis.
ResultsAt 1 month, mean ΔV was 3.36 ± 1.89 mL (right) and 3.13 ± 1.89 mL (left); all 17 cases showed ΔV > 0 (sign test p = 7.6×10⁻⁶) with no significant asymmetry. Among 12 patients with 3-month data, mean retention of ΔV was 92 ± 8% (right) and 91 ± 9% (left). Thread counts showed no significant correlation with ΔV. No infections or thread removals occurred.
ConclusionsNon-absorbable polyester–silicone thread lifting produced consistent upward volumetric lift with approximately 90% retention at 3 months and a favorable short-term safety profile.
