Background <p>Different facial rejuvenation techniques have been published. The evaluation of the results of these techniques is highly subjective. No technique remains the same in different patients, surgeons and even in the hands of one surgeon. Every patient presents a unique task for rejuvenation and harmonization based on the expectations of this patient and the end goal of the surgeon. The freestyle facelift technique herein presented aims to provide versatility in decisions and results following the basic pattern of deep dissection.</p> Methods <p>All patients who underwent facial rejuvenation with the freestyle facelift were included in the study. A retrospective review of the photos, videos and patient interviews during follow-ups was performed to evaluate the results. A 5 point Likert scale was used for the subjective evaluation of the results by the patients. The proposed technique involves a lateral facelift and temporal/forehead lift. Adjunctive procedures could be added in accordance with the patients’ preferences. Deep dissection creating the continuous plane from the temple to the neck is usually always performed. <i>Factors that determine the result</i> which are cardinal zones of elevation, traction points, fixation points, number of sutures, angles of traction and tension are manipulated differently to deliver patient specific results.</p> Results <p>The results of freestyle facelift concept were analyzed in 90 consecutive female patients (27–73 years old). The mean follow up period was 13 months. The rate of satisfaction was high (mean score was 4,6 using Likert scale) which shows that we were able to deliver personalized results. There were no major complications. 8 patients had transitory temporal branch paresis, 2 patients – slow blinking syndrome, all of which completely resolved by 3 months. 6 patients had small areas of ischemia behind the ear, hyperpigmentation after hemostatic net occurred in 2 patients and was visible for 2 months.</p> Conclusions <p>Freestyle facelift presents a versatile method for facial rejuvenation based on deep dissection which offers a possibility to tailor the technique to patients’ requests to achieve harmonious and individualized results.</p> <p><b>Level of Evidence:</b> Level III, therapeutic study</p>
