Contra-lesional hippocampus in low grade glioma patients with no hippocampal infiltration - a study of structural remodelling

Low grade gliomas (LGGs) are slowly growing tumours that induce neuroplasticity. Previous studies have shown that hippocampal invasion by an LGG may induce enlargement of the contra-lesional hippocampus, including its grey matter volume (GMV) and subfield volumes. However, it is not clear whether similar changes may occur in patients with an LGG that does not invade the hippocampus. The goal of this study was to assess GMV and hippocampal subfields of the contra-lesional hippocampus in LGG patients with no hippocampal involvement.
MethodsThis was a retrospective study. 3D T1-weighted magnetic resonance imaging scans of 30 LGG patients (17 left-sided, 13 right-sided) with no hippocampal invasion and 26 healthy controls were included. GMV was analysed using voxel-based morphometry, and hippocampal subfield volumes were analysed using FreeSurfer 6.0. In the analysis, we focused on regions reported in the literature to be enlarged in LGG with hippocampal infiltration.
ResultsContra-lesional hippocampal GMV was significantly larger in the left-sided LGG group (T = 7.78, p < 0.001) and the right-sided LGG group (T = 6.39, p < 0.001). A larger hippocampal-amygdala transitional area (HATA) volume was observed in the right-sided LGG group (Benjamini-Hochberg false discovery rate (FDR) adjusted p = 0.04, Bonferroni corrected p = 0.04). In this group, GC-ML-DG head (the granule cell molecular layer of the dentate gyrus head) was also enlarged after FDR adjustment (p = 0.04) but was not statistically significant after Bonferroni correction (p = 0.05).
ConclusionThe hippocampus contralateral to LGG may undergo structural remodelling even when the ipsilateral hippocampus is not affected by tumour infiltration.
